We have designed this plan for patients that do not have dental insurance and want an affordable solution to receive the best dental care.

Exclusions & Limitations


•  Membership fees must be paid in full prior to receiving benefits
•  All of patient-portion must be paid at the time of service or the usual fees will apply
•  No refunds after the annual fee has been paid
•  Offer is valid for 1 year of service from date of membership payment
•  Renewal rates subject to change
•  The FAMILY plan includes parents and their children to age 25

•  In conjunction with another dental plan
•  For services for injuries covered under workman's compensation
•  For treatment which, in sole opinion of the treating doctor, lies outside the realm of their capability
•  For referrals or specialists
•  For hospitalization or hospital charges of any kind
•  For cost of dental care that is covered under medical insurance
